Hepatopulmonary Syndrome: Bedside to Bench and Back Again

The hepatopulmonary syndrome is a unique complication of chronic liver disease, resulting from alterations in the pulmonary microcasculature, that lead to hypoxemia. Its presence increases mortality in cirrhosis and the only effective therapy is liver transplantation.
Over the last 20 years, increased recognition of the clinical problem and its adverse consequences has fueled translational studies that have now come full circle with the initiation of clinical trials of medical therapy.
