M/M/R $45.00 (Mumps, Measles & Rubella)
Twinrix (three injections) $110.00 each (Hepatitis A and B)
Gardasil (HPV Vaccine- three injections) $150.00 per dose

Students who are under 18 years of age must have written parental consent to receive immunizations. Visit "www.intravene.net/school_vaccine_consent.pdf" for the Informed Consent Form (PDF) to be filled out and mailed or faxed to Intravene.
