DON'T LET A BUG CATCH YOU!
Attend the fall immunization clinic featuring the INFLUENZA & MENINGITIS VACCINES! No appointment necessary! The clinic will take place from 11 a.m. to 8 p.m. in the Commonwealth Ballroom of Squires Student Center. Flu, Meningitis, and other vaccines will be available/given.
Fees:
Flu $25.00
Meningitis $110.00
Tetanus $30.00
Tdap $50.00
Hepatitis B (three injection series) $70.00 per dose based on age
Hepatitis A (two injection series)
M/M/R $45.00
Gardasil (three injection series) $150.00 per dose
Twinrix (three injection series) $110.00

Students who are under 18 years of age must have written parental consent to receive immunizations. Visit "www.intravene.net/school_vaccine_consent.pdf" for the Informed Consent Form (PDF) to be filled out and mailed or faxed to Intravene.
